The Radiant Light is spreading & we are growing! Radiant Hospice seeks a compassionate Hospice LVN experienced with providing direct in-home care to patients approaching end of life. Hospice LVN completes patient visits as assigned and coordinates with the IDG team to ensure continuity of care. The Hospice LVN makes visits to the patient's place of residence and facilitates the hospice patient's care. The Hospice LVN will collaborate with the other members of the patient's care team, attending physician, and patients and their families to coordinate quality care. Hospice LVNs evaluate a patient's physical condition and provides care to address any of the patient's medical needs. Apart from providing comfort to patients, RNs are adept at counseling and guiding patient's families as well.
